Remove unique key on file_thumbnail.url

We're getting "DB error: already exists" on thumbnails coming from
embed.ly. We don't need this to be unique, so let's avoid that.

This is my personal clone from the wonderful software GNUSocial. I may fix or break things here, so use it on your own risk. :-)
